Service Mission and Goal

To provide one-stop community support services for persons with disabilities and their family members / carers, to enhance the domestic living and community living skills for persons with disabilities so as to facilitate their integration into the community. It also provides training and support services to the family members / carers of persons with disabilities so as to strengthen their caring capacity and to relieve their stress.

 

Service Highlights

  • Case management service (only for resident living in Eastern or Wan Chai district);
  • Professional therapy services (only for resident living in Eastern or Wan Chai district);
  • Caring Angel ~ Individual support and holiday care services;
  • Day Respite Service;
  • Day Care Service for Persons with Severe Disabilities (Only available at WE NET Club(Eastern), target group is the persons with severe intellectual disability or severe physical disability aged between 15 and 59);
  • Individual or group training programmes and support services, to enhance their domestic living and community living skills;
  • Social, recreational and personal development programmes;
  • Training programmes / educational courses / workshops for family members / carers of persons with disabilities to enhance their caring capacities;
  • Supportive services to family members / carers of persons with disabilities to relieve their stress;
  • Dissemination of information on community resources and referral services for appropriate welfare services;
  • Community education and promotion of centre services; and
  • Drop-in services.

Target Group

  • Persons with disabilities living in the community, including persons with intellectual disabilities, persons with physical disabilities, persons with autism, persons with hearing impairment, and persons with visual impairment; and
  • Family members / carers of persons with disabilities.

 

Application

  • Application can be made to the service unit directly; or
  • Referral by other organizations.
  • Referrals can be made by social workers directly to a specific service unit (Applicable to Day Care Service for Persons with Severe Disabilities).

 

Fee

Charges according to the amounts specified by the Social Welfare Department. For details, please contact the service unit.

Monday to Saturday: 9:00am – 6:00pm

(Day Care Service for Persons with Severe Disabilities – Monday to Friday: 9:00am – 6:00pm)

Sunday and Public Holidays: Closed

 

Withdrawal

When the membership expires without applying for renewal, the membership will be deemed to be automatically withdrawn from the service.

Service Mission and Goal

The “Designated Team for Special School Leavers” (Designated Team) is part of District Support Centre (DSC) for Persons with Disabilities’ service. The Designated Team aims at providing care and support service for Special School Leavers to facilitate their smooth transition from school to another stage, and relieve stress of Carers.

 

Service Highlights

  • Case Management Service: formulate and follow-up short-term and long-term care plans, counselling and emotional support, home/school visits, outreaching support services, and matching community support services;
  • Early Intervention: seminars, workshops and activities for Carers;
  • Adaptative Programmes: self-development, social and recreational, and therapeutic group; and
  • Groups for Carers: mutual groups and training groups.

 

Target Group

Students/Graduates who are studying in/have graduated from Special Schools which are assigned to our DSC by Social Welfare Department, and meet the following conditions:

  • Day Students who will leave school in no more than 6 months and their Carers;
  • Boarding Students who will leave school in about 12 months and their Carers; or
  • Graduates who have left schools within 18 months and their Carers (w.e.f. mid-2024 or later).

 

Application

  • Refer by school social worker (recommend); or
  • Direct application to the Designated Team.

 

Fee

Charges according to the amounts specified by the Social Welfare Department.

Monday to Saturday: 9:00am – 6:00pm (Also provide services as needed)

Sunday and Public Holidays: Closed

 

Withdrawal

Request withdrawal from the service at any time.
