Service Mission and Goal

To provide one-stop support services for people with disabilities, their families and carers living in the Eastern District to strengthen their home and community life skills so as to help them integrate into the community. The service also provides training and support for family members/carers to improve their ability to take care of people with disabilities and relieve their pressure.

  • Drop-in service;
  • Community resources and other referral service; and
  • Day Care Service for Persons with Severe Disabilities.

Target Group

District Support Centre

  • People with disabilities living in the community (Eastern District);
  • Family members/carers of people with disability; and
  • Community at large.

Day Respite Service

  • People with intellectual disability and/or physical disability aged 6 or above.

Day Care Service for People with Severe Disabilities

  • People with severe intellectual disability or severe physical disability aged between 15 and 59; and
  • People within the same age range with mild or moderate intellectual disability who are assessed to have confirmed ageing conditions.

 

Application

  • Application can be made to the service unit directly; or
  • Referral by other organizations.

 

Fee

  • Annual membership fee ($21/ year)
  • Group and holiday care services (Details refer to WE NET CLUB Newsletter)
  • Individual support services ($33/hour, the minimum service hour of each service are 1.5 hours)
  • Day Respite Service ($5.1/hour, 1 hour as a unit)
  • Occupational therapy/Physiotherapy services ($52/ session, each session is 45mins)
  • Take the vehicle provided from centre ($10/a ride)
  • Day Care Service for Persons with Severe Disabilities ($1,002/month; including lunch/Contact WE NET Club for daily service fee)

Monday to Saturday: 9:00am – 5:00pm
(except public holidays)

 

Withdrawal

When the membership expires without applying for renewal, the membership will be deemed to be automatically withdrawn from the service.

The “Designated Team for Special School Leavers” (Designated Team) is part of District Support Centre (DSC) for People with Disabilities’ service. The Designated Team aims at providing care and support service for Special School Leavers to facilitate their smooth transition from school to another stage, and relieve stress of Carers. The Designated Team for Special School Leavers (Designated Team) is part of District Support Centre (DSC) for Persons with Disabilities’ service. The Designated Team aims at providing care and support service for Special School Leavers to facilitate their smooth transition from school to another stage, and relieve stress of Carers.

 

Service Highlights

  • Case Management Service: formulate and follow-up short-term and long-term care plans, counselling and emotional support, home/school visits, outreaching support services, and matching community support services
  • Early Intervention: seminars, workshops and activities for Carers
  • Adaptative Programmes: self-development, social and recreational, and therapeutic group
  • Groups for Carers: mutual groups and training groups

 

Target Group

Students/Graduates who are studying in/have graduated from Special Schools which are assigned to our DSC by Social Welfare Department, and meet the following conditions:

  • Day Students who will leave school in no more than 6 months and their Carers; or
  • Boarding Students who will leave school in about 12 months and their Carers; or
  • Graduates who have left schools within 18 months and their Carers (w.e.f. mid-2024 or later); and
  • Carers of above.

 

Application

  • Refer by school social worker (recommend); or
  • Direct application to the Designated Team.
